The purpose of this dataset is to securely store patient experience survey data for the Patient Assessment of Chronic Illness Care and Modified Shared Decision-Making Questionnaire.
Need for Personal Health Information
The PHI is required by Regional Renal Programs to inform about the experience of renal care within their program and are used to drive local improvements to care.
